چکیده

Stress cardiomyopathy has an association with emotional or physical stressors and is found to be more common in postmenopausal women. It has a similar presentation as acute myocardial infarction. Involvement of apex at the initial presentation and recurrence at the mid ventricular segment is rare. We describe a case of a 46 year old female with apical involvement at the initial presentation but mid ventricular involvement at the recurrence. 46 year old woman developed sharp chest pain while giving awards at an honors assembly. She was brought to the ER and was found to have a troponin level elevation without any EKG changes. Cardiac cath did not show coronary artery disease. ECHO revealed a low EF of 30 % with apical ballooning. Patient's symptoms resolved and her EF normalized on repeat ECHO. 2 years later patient developed similar symptoms. She had a troponin elevation without EKG changes. ECHO revealed mid-ventricular ballooning with a low EF. Patient's symptoms again resolved and repeat ECHO showed a normalized EF. Our patient had two different episodes of stress cardiomyopathy. Recurrence rate has been reported up to 11.4 percent in previous studies (5). This case is unique as recurrence was at different anatomic locations i.e apex followed by the mid ventricular segments. Further research is needed to understand the underlying mechanisms associated with the stress cardiomyopathy as current hypothesis do not completely explain

A Unique Case Of Recurrent Takotsubo Cardiomyopathy- Atypical Followed By Typical Variant

Takotsubo cardiomyopathy (TC), synonymous with apical ballooning syndrome, broken heart syndrome, stress induced/ ampulla cardiomyopathy is characterized by transient left ventricular dysfunction of apical ( typical) or mid segments (atypical), mimicking acute myocardial infarction in the absence of significant coronary artery disease . We report a rare case of recurrent takotsubo cardiomyopath...

Midventricular form of takotsubo cardiomyopathy as a recurrence 1 year after typical apical ballooning: a case report

Takotsubo cardiomyopathy was first described in Japan and is characterized by transient left ventricular apical ballooning in the absence of a significant coronary artery disease.Caused by the clinical presentation including chest pain, electrocardiographic changes and elevated myocardial markers this syndrome is frequently misdiagnosed as an acute coronary syndrome. Recurrences of Takotsubo Ca...

Dobutamine stress echo-induced apical ballooning (Takotsubo) syndrome.

AIMS We report a case of dobutamine stress echocardiography (DSE) resulting in transient apical ballooning syndrome to highlight this rare condition as a potential complication of DSE. BACKGROUND Takotsubo cardiomyopathy, or transient apical ballooning syndrome, is a recently described form of left ventricular (LV) dysfunction induced by stress. Clinically it can mimic acute coronary syndrome...

Apical ballooning with mid-ventricular obstruction: the many faces of Takotsubo cardiomyopathy

Takotsubo cardiomyopathy (TTC) is a transient left ventricular dysfunction due to akinesia of the left-ventricular (LV) mid-apical segments (apical ballooning), which can cause severe reduction in LV systolic function. The typical clinical picture of TTC include chest pain, electrocardiographic changes consisting of mild ST-segment elevation followed by diffuse deep T-wave inversion, QTc interv...

Tako-tsubo cardiomyopathy

Tako-tsubo cardiomyopathy (transient left ventricular apical ballooning) is a reversible form of cardiomyopathy of unknown etiology. Tako-tsubo Cardiomyopathy (TTC) is typically precipitated by sudden emotional or physical stress, and is associated with excessive sympathetic stimulation and catecholamine release. Its clinical presentation is similar to that of acute coronary syndrome. The diagn...
